Mine went bad too. It's a “Universal Ignition Switch” ACDelco Part #D1460 (#12339178) but with a custom bezel. The bezel actually costs more than the switch. You have to file down the bottom of the porcelain "triangular" separator for it to fit behind the dash lip. Other than that, pretty straight-forward replacement. I've had no problems with the replacement; the original was twitchy from the get-go.
