If my pic attached to this message, is this the same as your shifter? If so, I don’t use any grommets or washers under the shift lever. The washers were probably used to change the orientation of the shift lever, if not required, eliminate them. Also, the grommets are used for minimizing vibration at the shift lever, not required if not important to you. If you eliminate the washers and grommets all you need are lock washers and to tighten the fasteners properly and the lever will remain secured. If for whatever reason you wish to keep the washers and grommets, a liberal amount of blue thread locker on the fasteners should keep them in place and secure.
