stamped steel clutch forks can, and do, fatigue and bend. I've had it happen to me before.
I installed a used fork with a heavy duty Zoom clutch and every time I would adjust it, I could only depress the pedal a few times before I'd have to readjust it again. After several attempts, I finally pulled the pedal out and it looked good at a glance, but upon close examination, I could see the tiny little stress cracks which allowed it to bend.
