The C6 to start the number is a 1966 engine. I don't have my papers here at the moment to tell you what the AZ means but it most likely will tell what car or model the motor would have been in. The NOS as I am sure you know means New Old Stock. It was a new motor that was never used. Some of the guys can give you a link that will tell you almost every breakdown of the codes. I have it in the book but as I said, I don't have my book here. I am almsot sure that they never made any 428 CJ engines after 1969.
