I think the indents in the washer are suposed to go towards the bush as they are designed to grip the crus tube and stop the bush rotating. If you are using nolathan bushes the indents don't matter so much as the bush runs in grease and is designed to slip.

From memory I'm running the bolt through the bush then a washer and then a 6mm aluminium spacer. Through the cross member then another washer the bush another washer then the castle nut. The nut still rubs on the rack gator but not enough to be a problem.
