Actually, it sounds like either the linkage had/has come loose or the shifter handle has come loose. It does not sound like the trans.
IMHO, you need to put loctite blue on a small portion of the threads for the shifter and the rods. Blue will allow you to remove the nuts and bolts, but act like a Nylon washer behind the nut(s).
