Thanks. I had your mailing address and phone number but that page has came out of the loose leaf note book and I can't find it anywhere. In fact just looking I am going to have to do several pages over as I never put the re-enforcement rings around the holes and over the years opening and closing the book has caused a couple of pages to just barely be hanging in there.
