I've heard of it happen, that's what happens when they go bad.
This is what happened to the one I replaced mine with on my 5.0. Needed one quick so i just went to autozone and grabbed the best one they had with a warranty. the old one was on the crank tight, put the new one on and it wasn't really snug at all. It wasn't wobbling around, but it just slide right onto the crank. I didn't think too much about it, had to get the car put back together so I could drive it to work in the morn. It ran great for a few weeks then, coming home from work it just went out. Car was shaking violently!!!! Get home and pull the balancer off to find this....
Notice... MADE IN CHINA!!!!!!! Anyways, i took it back to autozone and wanted a replacement. It had a LLT warranty. the guy at the desk accused me of improperly installing it. Lifted his shirt and showed me his Mustang tattoo and basically called me an idiot! He exchanged it after I threw a fit in the store and I was on my way. Put the new one on and it fit the same way. I took it to Orielly's and had them Mic it. It was 125 thousandths bigger in diameter than the original. They recommended I take it back. Autozone refused it. I called and talked to their manager and he said to bring it back and they'd exchange it. Every single one they had was the same. I ended up having my friend exchange Autozone's part for theirs and it worked perfect! I've never shopped at autozone since!
Moral of the story... make sure it fits properly or it will keep going bad!
