First thing to check is the shifter boot clearance. Often if the rubber boot is used it is so stiff it can force the transmission out of gear when letting off the throttle.
Just remove the boot & ring & go for a short drive to check it out. No matter what the boot is made of this is the best & easiest place to start as it also allows you to check the centering of the shifter in the cutout.
