I have a different Lakewood bellhousing since it goes behind a small-block Chevy in my Lister but the response that I got back from the Lakewood tech department when I emailed them was to "make one". Not exactly what I wanted to hear but after trying everything that I could find from GM and nothing was close I did make one. It was easy in my case since I have a hydraulic throw-out bearing and all I needed to do was to cover the hole to keep dust out and allow the lines to run through it. Probably not as easy with an actual clutch fork but it may be do-able. Good luck.
