Default Clutch Fork S/B

I have a used clutch fork for small block 289/302/351 application. It is unmolested and in very good condition without any rust or damage, painted black, included is the retaining spring and a NEW OEM dust boot. This is most commonly used with a "Long" style pressure plate, but may be adaptable to others. This is the type that pushes/pulls to the rear (outside the housing) to disengage the clutch.

$45.00 + actual shipping.

Do you have a part number? Never heard of that style clutch fork, but would be interested in finding out more about it, especially if it will work with my Lakewood bellhousing.

Is the clutch fork for 60's factory style bellhousing?
Where did you get slave cyl and bracket?
I need that setup for kmp218X.
