Any '84 up 302/5.0 Mustang T-5 equipped bellhousing will fit the 351W. Just replace the fork and fulcrum with new ones.

You don't want a flexplate, they're for automatics. T-5's use a flywheel. Do yourself (and your legs) a favor and get a new one, billet steel, 157 tooth, 28in/oz balance. Production flywheels are cast iron and can come apart at high rpm.

Same for the T-5 tranny. All production Mustangs used 3.37 low to give the 2.73 rear geared cars some acceleration. Their torque rating was not that good, and they have even less of a lifespan behind 351's. The 2.95 low world class units are a lot stronger but you have to get them new. I got mine from Liberty Transmissions who blueprinted the unit, and installed a .80 overdrive gear. At 75 MPH, the engine is ticking over at 2500 RPM.
