Default Cluch servo retract arm brakes off.

I have a 1966 shell valley with a 289/ 4 speed top loader. About every 6 months the pull slave servo arm brakes off where the arm passes through the chluch fork. I have made the hole larger and a thin layer of teflon grease on the rounded locking bolt. The alignment looks good with no side loads or binding.
Does any one have any tips. I just replaced with a new one.

I modified my arm by cuting off the area where the hole was and then welding a piece of 1/2" round stock about 1 1/2" long, then cut a slot in it, thread one side and drill the other and use a high quality 5/16" female Heim joint. Works perfect.
